Briefly losing your way, literally or figuratively. For example, due to dementia, addiction, an intellectual disability, or a mental health issue. Or simply because you are temporarily unable to express yourself. The Help Card is designed to quickly provide these individuals with the help or care they need at that moment. It is a small, foldable card the size of a bank card. The card states what people who receive the card can or cannot do, and who the contact person is. In this way, the Help Card offers the cardholder the opportunity to maintain control in situations where they are temporarily unable to express themselves. And the Help Card provides support to people who want to help others but don't always know how.

Would you like to request a Help Card?

Are you or do you know someone who might also feel more confident with an Assistance Card in their pocket? If so, you can request the Hulpkaart from the municipality's Area Team. Call 14 0511 or send us a message via the contact form. More information about the Hulpkaart can be found on the Hulpkaart website.

Collaboration

From February 2020 to February 2021, the municipalities of Achtkarspelen, Tytsjerksteradiel, and Waadhoeke are working together with GGD Fryslân, Stichting AanZet, and MEE Friesland in the Hulpkaart Fryslân project. The common goal is to raise awareness, ensure support, and stimulate the use of the Hulpkaart. The project is made possible by a subsidy from ZonMw.
