Wmo Calamity Report
Providers of social support in the Netherlands are required to report calamities or violence to the Wmo Supervisor of the municipality.
Reporting emergencies and incidents provides insight into what is going wrong. By analyzing what went wrong and whether and how this can be prevented in the future, an organization learns from emergencies and incidents. That is why care providers are obliged to report emergencies to the supervisor and to register and analyze incidents themselves. Calamities must also be reported from Area Teams. In the event of a calamity, the supervisor investigates whether the care provider has acted adequately, whether the quality of a facility and the safety of clients are guaranteed, and whether improvement measures are necessary.
How does it work?
Calamity or violence
All calamities and incidents of violence during the provision of a service must be reported to the municipality's Social Support Act (Wmo) supervisor within 3 working days of discovery.
A calamity is “an unintended or unexpected event that relates to the quality of a service and that has led to a serious harmful consequence or the death of a client.” Violence during the provision of a service: sexual penetration of the body of or indecent assault with a client, as well as physical and mental violence against a client, by a professional or by another client with whom the client stays in an accommodation of a provider during the 24-hour period or part of the day.
A calamity includes:
- Any unexpected and unintended death of a client;
- Any suicide (attempt) by a client that may be related to shortcomings in care, for example, due to insufficient supervision, poor communication, or errors during transfer;
- An event that has led or could lead to serious and/or permanent physical injury to a client;
- An event that has led or could lead to serious mental suffering of a client;
- Serious transgressive behavior: physical, psychological and/or sexual by aid workers or other clients.
Violence can include serious transgressions, whether physical, psychological, or sexual, by a professional or another client. Please contact the Wmo contact person at 14 0511.

Handling report
After receiving the notification, the municipality will contact the person who reported it and the organization where the incident took place. Depending on the severity of the situation, the municipality may ask the provider to investigate the incident or conduct its own investigation. A report will be drawn up of the investigation. The report will include whether, and if so, which improvement measures should be taken.
The purpose of the calamity supervision and the investigation of calamities is to improve the quality of the support offered.
