Calamity notification Wmo

Providers of social support in the Netherlands are required to report calamities or violence to the municipality's Wmo supervisor.

Reporting calamities and incidents provides insight into what is not going well. By analyzing what went wrong and whether and how this can possibly be prevented in the future, an organization learns from calamities and incidents. That is why care providers are required to report calamities to the supervisor and record and analyze incidents themselves. Calamities must also be reported from Area Teams. During emergency supervision, the supervisor examines whether the care provider has acted adequately, whether the quality of a facility and the safety of clients is guaranteed and whether improvement measures are needed.

How does it work?

Calamity or violence

All calamities and incidents of violence in the provision of a facility, must be reported to the municipality's Wmo supervisor within 3 working days of discovery.

An emergency is "an unintended or unexpected event, relating to the quality of a facility, that resulted in a serious harmful consequence to or death of a client." Violence in the provision of a facility: sexual intrusion into the body of or fornication with a client, as well as physical and mental violence towards a client, by a professional or by another client with whom the client is staying in a provider's accommodation during the 24-hour or part of the day.

A calamity includes:

  • Any unexpected and unintentional death of a client;
  • Any suicide (attempt) of a client, possibly related to deficiencies in care, for example, because there was inadequate supervision, poor communication or errors in transfer;
  • An event that has resulted or may result in serious and/or permanent physical injury to a client;
  • An event that has resulted or may result in serious mental suffering of a client;
  • Serious transgressive behavior: physical, psychological and/or sexual by caregivers or other clients.

Violence can include serious transgressive behavior whether physical, psychological or sexual by a professional or other client. Please contact the Wmo contact person by calling 14 0511.

Handling notification

After receiving the report, the municipality contacts the reporter and the organization where the calamity occurred. Depending on the seriousness of the situation, the municipality may ask the provider to investigate the calamity or conduct its own investigation. A report is drawn up of the investigation. The report will include whether, and if so what improvement measures need to be taken.

The purpose of emergency monitoring and investigation is to improve the quality of support offered.
