Change of last name
- If you want to change your last name or that of your child, first check in the brochure Naamswijziging (Name Change) from Justis whether this is possible. Justis is the screening authority of the Ministry of Justice and Security.
- If your situation is described in this brochure, you can apply to Justis for a name change.
How does it work?
- Download the application form and fill it in.
- Check the Name Change brochure to find the code that applies to your situation. Enter this code on the application form.
- Add the requested documents to the form and send everything to Justis.
You will automatically receive a message whether your last name will be changed. You will then receive an official excerpt from the Royal Decree.
Adjusting documents
- If your last name has been officially changed, this must also be changed in your identity document. Therefore, apply for a new passport, identity card and/or driving license.
- Also, make sure you inform all relevant authorities about your changed name. You do not need to inform the municipality separately. They will automatically receive an extract of the Royal Decree.
