A certificate of life is an official statement confirming that you are alive. You can request a certificate of life from the municipality where you are registered. 

Request proof of life

You can request a proof of life certificate in 2 ways: 

  • You can request the statement online using the button below. You pay immediately via iDEAL. We will send it to you by post. You do not have to come to the town hall. 
  • You must make an appointment to complete the entire application at the town hall. You pay for the declaration during your appointment. You can take the document with you immediately.

How much does it cost?

A certificate of life costs € 17.10.

The proof of life certificate is free if you can demonstrate that you need it for your pension. Please bring your pension statement with you.

What do I bring to the appointment?

Are you applying for proof of being registered in person at the town hall? Then bring the following with you: 

  • a valid proof of identity; 
  • money or a bank card. 

Are you requesting the declaration for a pension fund? Then you also need the original letter from the pension fund. 

Applying for a certificate for your minor child(ren)?

Parents can request the statement for their minor children if they are registered at the same address.

Where is it valid?

You may need a certificate of life if your life insurer or pension fund requests it. A certificate of life is valid in: 

  • The Netherlands 
  • Aruba 
  • Curaçao 
  • Bonaire 
  • Sint Maarten 
  • Sint Eustatius 
  • Saba
