Change of last name usage

If you are married, have entered into a registered partnership, are a widow or widower, or are divorced, you can have the use of your last name changed by the municipality. You can then choose:

  • only your own last name
  • only your partner's last name
  • the surname of your partner followed by your own surname
  • your own last name followed by your partner's last name

Please note! This is not the same as changing your own last name because you want the last name of another parent or because your last name has a strange meaning.

When you register your marriage, you can choose how your name is used in public life. This is done by signing a "declaration of name use."

Submit in writing

You can also choose to submit your choice to us in writing. Download the form, sign it and send it to Gemeente Tytsjerksteradiel, Postbus 3, 9250 AA Burgum.

You can also bring the form to the appointment for the notice of intent to marry or submit it in person at the town hall.
